Clinical & NDIS Reports

Advocacy, Documented.

Navigating the NDIS can be complex, and having the right documentation makes all the difference. Our therapists prepare clear, thorough reports that accurately reflect your functional needs, support goals, and the reasonable and necessary supports you require. Whether you're entering the scheme, preparing for a plan review, or appealing a decision, we're here to make sure your voice is heard.

Example Reports

Our clinicians have a thorough knowledge of reporting requirements for the NDIS and broader healthcare landscape. They are skilled in communicating clinical concepts in an accessible and goal-oriented manner. Our reports efficiently summarise your needs and outline recommendations that are relevant to your goals.

  • A report summarising your initial presentation, physical needs, and therapy plan.

  • A report summarising your physical needs, therapy recommendations, and funding quote for continuation of services to support NDIS plan re-assessments.

  • Reports for Mid- and High-cost assistive technology following NDIS templates and guidelines.

  • A clinical document outlining the support and equipment needed to help keep you safe in your home when transferring and mobilising.

  • A letter which outlines clinical needs to support medical or NDIS applications.

 FAQs

  • If you're applying for the NDIS, preparing for a plan review, requesting new or increased funding, or appealing a decision, a clinical report from a therapist can significantly strengthen your case. Your NDIS planner or support coordinator may also recommend a report as part of your planning process.

  • Turnaround times vary depending on the type and complexity of the report. A support letter may be prepared within a few days, while a comprehensive functional capacity assessment report may take one to two weeks following the assessment. If you have a deadline, let us know and we'll do our best to accommodate it.

  • Yes. We encourage participants to review their report before it's finalised to ensure it accurately reflects their situation. If anything doesn't seem right, we'll work with you to address it.

  • Yes. A well-prepared clinical report can be a valuable tool in requesting an internal review or submitting to the AAT (Administrative Appeals Tribunal) if you disagree with an NDIS decision. Our clinicians are experienced in preparing reports that clearly articulate functional need and support justification in these contexts.
